Independent Verification Form (2011-2012)
Your FAFSA might be selected for review in a process called "Verification" as required by the Federal Government. If so, an Independent Verification form, along with your 2010 Federal Tax form and W-2(s) will be requested from you.

Institutional Charges (2010-2011)

♦ Return of Title 4 Loan Funds

Loan funds can be returned within 120 days of disbursement. Complete this form in order to initiate the loan return process.

Consortium Agreement Request Form

Planning to visit another school for a semester or two? Simultaneous to requesting approval to visit away from Dean Niedzielko of the Academic Affairs Office, you should complete and submit this form to the Financial Aid Office. Financial Aid from CUA may be available to finance your semester away.
